Application Instructions

Proposal Submission
Proposals are due by 5 p.m. on Friday, June 29. Please provide one complete original and nine complete copies of the proposal. No faxed or emailed proposals, please.

Proposal Elements
Please include all of the following elements in your proposal:

I.

Cover letter.
One page on organization letterhead with:

  • Amount of endowment goal
  • Reason for applying for the Great Giving Match now
  • County where the organization is located and county(s) and city(s) served
  • Address, telephone, e-mail address, and fax number
  • Project director name and title
  • Signature of Executive Director, President/CEO or Principal
  • Signature of Board of Directors (or governing body) President
II.

Agency information.
Please address the following in 1-2 pages:

  • Mission statement and brief statement of organization’s goals and/or objectives.
  • Brief summary of organization’s history including founding date.
  • Description of current programs, activities and clients served.
  • Affiliations with similar organizations
III. Financial information sheet.
Use the financial information sheet to describe your organization’s financial and fundraising history.
IV.

Narrative.
Please address the following in no more than 3-4 pages:

  • What is your organization’s endowment goal for the Great Giving Match? Explain how you arrived at this amount. Your endowment goal may be any amount between $5,000 and $25,000. This is the amount that your organization will raise each year for two years (2002 and 2003). If your organization is selected and you are successful in raising the amount you choose, The Community Foundation will match this amount both years. Example: your endowment goal is $10,000. You will raise a total of $20,000 and receive a $20,000 match, for a total endowment of $40,000 after the two-year program.
  • Why is having this endowment significant to your organization? Why is this the right time for your organization to take this project on? How will it meet your organization’s long-term needs?
  • Does the organization currently have an endowment? If so, what is the current market value? How was the endowment raised? How much income does it provide annually, and how are those funds currently used? How is the endowment managed?
  • Describe your goals for ongoing endowment growth. Explain how your organization will incorporate endowment fundraising into its future fundraising activities.
V. Endowment goal outline sheet.
Use the endowment goal outline sheet to describe your plans for meeting your endowment goal for the Great Giving Match.
VI.

Attachments.
If you have submitted any of these items in a grant application to the Community Foundation within the last calendar year, and the information has not changed, you do not need to resubmit that item.

  • Board of Directors (or other governing body) and their occupations and/or community affiliations.
  • Anti-discrimination statement.
  • Names and qualifications of key staff, including person responsible for the Great Giving Match campaign.
  • Financial statements or balance sheet.
  • IRS tax-exemption letter.
  • Most recent annual report.
  • Annual budget for current fiscal year.
