Grant Application
Nonprofit, governmental or
other public groups interested in applying for grants
from the Millennium Trust are invited
to submit a concept letter describing the proposed
project. The Millennium Trust Advisory
Committee will review the concept letters and choose
a number of finalists to submit a full application (as
described below).
Please supply:
- COVER LETTER One page which addresses
the interests and specific priorities of the funding
source and amount requested.
- SUMMARY OF
APPLICANT ORGANIZATION (This form is available
online or by calling our office at 303-442-0436
(toll free: 1-877-744-7239)).
Note: This Organization Summary is also available for download in Microsoft Word format; you can download it and fill it in on your own computer.
- NARRATIVE (preferred length
not to exceed three pages) Include the following information
in any order.
- Agency Information
- Mission statement, brief statement of organization's goals and/or objectives;
- Brief summary of organization's history;
- Description of current programs, activities and accomplishments.
- Purpose of Grant:
- Brief statement of the issue to be addressed; description of the constituency served (include number served); target population; how will they benefit?
- Description of goals and objectives for the purposes of this grant;
- Description of activities planned to accomplish these goals; is this a new or ongoing activity on the part of the sponsoring organization?
- Timetable for implementation (if for specific program or capital project);
- Other organizations, if any, participating in the activity;
- Long term sources/strategies for funding at the end of the grant period.
- Strategy for evaluating the program/organizational
outcomes.
